If you're living with a serious illness such as cancer, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), or Parkinson’s disease you can count on Capital Region Palliative Care and Hospice to partner with you, your loved ones, and your doctor to address your physical, emotional, social, and spiritual needs with the goal of improving your quality of life.

As a collaboration with Concord Regional Visiting Nurse Association our palliative care core team includes a palliative care physician and/or nurse practitioner, nurse, social worker, and spiritual counselor. Together they work to improve symptoms such as:

  • Pain
  • Shortness of breath
  • Fatigue
  • Constipation
  • Nausea
  • Loss of appetite
  • Difficulty sleeping
  • Depression and stress

What Our Palliative Care Team Does for You

  • Identifies goals by honoring and respecting your values, wishes, and priorities
  • Helps you in understanding your illness, prognosis, and treatment options
  • Supports you in making difficult medical decisions
  • Provides supportive counseling
  • Identifies your interests and arranges activities related to those interests
  • Provides help in navigating the healthcare system
  • Assists in coordinating treatments from different healthcare providers
  • Helps in advance care planning

Difference Between Palliative Care and Hospice Care

Hospice care focuses on comfort care for a patient’s remaining weeks or months at end of life. To qualify for hospice, a physician must certify that a patient has a terminal illness and has six months or less to live.

Palliative care provides an extra layer of support that does not necessarily exclude treatments aimed at reducing or curing an illness, and in many cases, is used to compliment treatments.
