Medical Home Navigation ProgramOur Medical Home Navigation Program's Nurse Navigators help you and your family navigate today's increasingly complex health system with keen focus on building trusting relationships between you and your healthcare team.

Your Nurse Navigator works in conjunction with your Medical Home team to assist with many components of your medical care, including:

  • Answering questions you may have about your care or medications;
  • Listening to your concerns or fears;
  • Communicating with the providers on your healthcare team including your primary care provider;
  • Keeping your family informed and engaged in your care;
  • Providing one-on-one education aimed at improving your health;
  • Helping you schedule needed medical appointments;
  • Arranging for additional community resources if needed, including home health, skilled nursing facilities, rehabilitation services and behavioral health.

Your Nurse Navigator helps you by:

  • Serving as a resource within the primary care team to partner with you as you manage transitions of care, which may include specialist to specialist, hospital to home, hospital to nursing home or rehabilitation;
  • Developing a plan of care and wellness that respects your medical needs and values;
  • Helping you manage your medications;
  • Listening to and understanding your fears and concerns about treatment;
  • Providing one-on-one education aimed at improving your health.
