Concord Hospital has just entered into the most significant transformation in our history to a single organization wide electronic health record. With this transformation, your providers now have one source of information to help them provide the best care, from the right provider in the right setting. The information available in My Patient Connect derives from your electronic health record.

To ensure all the information you see in My Patient Connect is accurate, your provider will review it with you at your next visit. Once affirmed, your allergies, problems and medications will be visible to you in My Patient Connect. If you have an immediate need, please call your Concord Hospital Medical Group practice. My Patient Connect is our NEW patient portal designed to engage you and your family in your healthcare by providing online access to information and tools to help you and your family take an active role in the healthcare decision making process.

First Time User

If you are at least 18-years-old and a Concord Hospital or Concord Hospital Medical Group patient you can request an invitation to enroll in My Patient Connect. Once you receive your invitation, you can register online.

Proxy Access

Proxy access is the official name for gaining access to another person's account. Parent's may request proxy access to their minor child's health information. Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course.

Invitation Requests

We are diligently working to process My Patient Connect invitation requests. If you've requested an invitation, you'll receive electronic confirmation once your enrollment is complete.
