First Time User

If you're at least 18-years-old and a Concord Hospital or Concord Hospital Medical Group patient you can request to enroll in My Patient Connect. Once you receive your invitation in your email account, you can register online.

Proxy access is the official name for gaining access to another person's account. Parents may request proxy access to their minor child's health information. Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course.
