In 2015, Concord Hospital and Concord Regional VNA launched Capital Region Palliative Care and Hospice to provide care for patients dealing with life-changing illnesses or medical conditions. Our interdisciplinary team includes physicians, nurse practitioners, nurse navigators, social workers and spiritual care counselors focused on relieving pain, stress and other debilitating symptoms of serious illness while working in partnership with each patient’s primary care provider, family and community resources. Palliative care can be provided in the Hospital, at a patient’s primary care office, a skilled nursing facility or in the home and often is delivered in conjunction with chronic disease treatment. Each patient’s palliative care team is focused on helping patients gain the strength to carry on with daily life by providing clear communication, practical information, critical thinking and compassion, while always keeping patient goals at the center of care.

The organizations are sharing the expertise of Dr. Stephen Rust, medical director of Concord Hospital Palliative Medicine and Hospice Care, to further develop Capital Region Palliative Care and Hospice and strengthen Concord Regional VNA’s existing Hospice Program.
