Partnering to Help Prevent Heart Disease

Healthcare providers at Concord Hospital and Concord Hospital Medical Group partner with the specialists at Concord Hospital Cardiac Associates to help prevent heart disease – for good reason. Small changes in diet, exercise and stress management can make a big difference in preventing the number one cause of death in the United States.

At Cardiac Associates, patients with potential heart disease routinely ask how they can make their condition go away and how can they prevent it from coming back. For those patients, prevention efforts might focus on how to thwart recurrences of cardiac conditions after treatment.

For other patients, every visit to their CHMG primary care physician includes counseling and screenings designed to prevent heart disease in the first place. Providers focus on screenings to help prevent or control high blood pressure, diabetes and high cholesterol. They also speak with patients about smoking, healthy eating, regular exercise, healthy weight and managing stress.

CHMG providers also refer patients to other resources, such as Freedom From Smoking, a smoking cessation program and to nutrition, weight loss, exercise, diabetes, and anti-stress programs available through Concord Hospital Center for Health Promotion.
