Center for Health Promotion Closing
Concord Hospital Center for Health Promotion, located at 49 South Main Street in Concord, is closing as a stand-alone facility on July 13, 2020, as part of work to implement a new community health and wellness model that makes more efficient use of system and community resources. Concord Hospital will continue providing programming that supports the identified health needs of community members as well as the Hospital's population health initiatives and will create new community partnerships to support such efforts. These continuing programs and services include diabetes education, nutrition consultation, cardiac rehabilitation, pulmonary rehabilitation, cancer wellness, parenting support and smoking cessation.

Whether you're seeking to lower your individual health risk, improve your quality of life, find support, prepare for a major life change like starting a family or retiring, we've got education classes, programs and workshops to help you live your best life. Classes are offered through our Payson Center for Cancer Care, Diabetes Self-Management Education program, Rehabilitation Services Department and The Family Place, as well as through several of our community partners.
