Community Members Encouraged to Participate in 2026 Concord & Lakes Region Community Health Needs Assessment Survey

CONCORD, NH – Community members across the Capital and Lakes regions are encouraged to share their voices by participating in the 2026 Concord & Lakes Region Community Health Needs Assessment Survey, now open online.

The survey, conducted in partnership with regional health care and community organizations, gathers important feedback on residents' health needs, priorities, and challenges. The results will help guide future programs, services, and investments to improve the health and well-being of individuals and families throughout the region.

“Community input is essential to understanding the needs of the people we serve,” said Gwen Whitney-Gill, Concord Hospital’s community health coordinator. “This survey gives residents an opportunity to directly influence how health care and community resources are developed and delivered in the years ahead.”

The survey is part of a broader Community Health Needs Assessment process, which helps organizations identify key health issues, address disparities, and strengthen access to care. Topics include access to healthcare services, mental and behavioral health, chronic disease, substance use, housing, transportation, and other factors that impact overall health.

All responses are confidential, and the survey is designed to be quick and easy to complete. Community members from all backgrounds and experiences are encouraged to participate.

“Every response matters,” Gwen added. “The more people who participate, the more accurately we can understand our community’s needs and work together to build healthier communities.”
