Concord Hospital Transfer Center Eases Patient Transfers from Other Hospitals

Each year, nearly 2,000 patients from hospitals around New Hampshire are transferred to Concord Hospital to receive specialized, advanced care.

To ensure that the process goes smoothly, Concord Hospital established a Transfer Center. The Center's goal is to coordinate transfers with a single phone call involving personnel at the Hospital who can connect the referring physician to the appropriate specialist, while also making sure the Hospital has the resources available to accept the patient.

"Concord Hospital receives approximately 2,300 transfer requests a year from other hospitals or physicians, an increase of about 15 percent in recent years — and a number that is expected to continue to increase," said Dr. Christopher Fore, Concord Hospital's Chief Quality Officer. The Hospital accepts approximately 80 percent of the transfers. Others are referred elsewhere when the Hospital is full.

The requests are for patients needing urgent transfer for conditions including heart attacks, stroke or unstable trauma, as well as for patients whose doctors would like them admitted to Concord Hospital for other medical and surgical conditions.

The Transfer Center operates around the clock, with access to information providing a complete understanding of the Hospital’s status when a transfer request is called in.

The information includes how many nurses are available to care for admitted patients, how many patients are being treated in the Operating Room or being treated or waiting in the Emergency Department (ED), how many others are on the way to the Hospital by ambulance or for scheduled procedures and how many beds are available.

Requests are coordinated through the Transfer Center, with standardized procedures to receive information, determine the Hospital’s ability to accept the patient and start the process. The Center will alert appropriate Hospital departments that a transfer is imminent or work to schedule the transfer at a more appropriate time in non-emergency cases.

Concord Hospital anticipates using the Transfer Center approach for patients being referred from a local physician’s office for a specific test, an ED visit, inpatient admission or other services.

“We want to make it easier to transfer patients,” Dr. Fore said. “We want the process to be about taking care of the patient in the safest and most efficient manner.”
