One health record
One sophisticated electronic health records system ensures continuity of care for our Concord Hospital Medical Group and Dartmouth-Hitchcock Concord patients by keeping family medicine providers informed of the care we provide their patients.
One-on-one care and attention from experienced providers are what you should expect at every visit. Our providers are all linked electronically and all within one hospital centered on innovative care.
