When calling for your films or images:
  • You need to give your full name and date of birth;
  • You are asked to fill out an image request form.
To Pick-up CD images:
  • You need to pick up your CD in Diagnostic Services;
  • You need to show a valid ID and sign a release form at time of pick up.
To have images sent to another facility or office via mail:
  • You need to give your name and date of birth;
  • The full name (in care of) and address to where the images are to be sent.
Mammogram films:
  • The Library needs at least 24 hours notice for mammogram films.

The Concord Hospital Health Information Management Services Department handles requests for reports.
