Teen Care at the Concord Hospital Family Health Center - Concord offers comprehensive health services to Merrimack County area youth between the ages of 11 and 21. The Family Health Center - Concord is located in the Yeaple Building adjacent to Concord Hospital.
Procedures Performed or Problems Diagnosed and Treated:
Teens who are primary care patients of the Family Health Center - Concord may receive the following services with parent/guardian permission.
- Preventive care
- Acute (sick) visits
- Physicals (including sports, camp, college)
- Chronic disease follow-up
- Immunizations
- Nutritional assessments and support
- Mental health counseling
- Eating disorders assessment
- Health education
- Attention deficit disorder (ADD) evaluation and follow-up
- Confidential substance abuse referrals, family planning services, and testing and treatment for sexually transmitted diseases and HIV. (Note: Teens may receive these services without parent/guardian permission although it is strongly encouraged.
Teens who are not primary care patients of the Family Health Center - Concord may receive the following services:

Teen Care is a caring and welcoming environment staffed by professionals who have an interest in and commitment to assisting youths. The multidisciplinary team, which represents family practice and adolescent medicine, nursing, mental health, nutrition, and health education, works together to provide comprehensive health services to young adults. Also, fees are based on the patient's income and family size. A teen receiving confidential services pays according to his/her own income.

Monday 3 p.m. to 6 p.m. (Walk-ins are welcome between 4 p.m. and 5 p.m.)
