The Concord Hospital Family Health Center - Concord and its satellite, Concord Hospital Family Health Center-Hillsboro-Deering provide primary and preventive care for the entire family in collaboration with NH-Dartmouth Family Practice Residency. As a state-supported community health center, the health centers provide a full range of primary care services regardless of ability to pay.

These and other services are provided to people in Concord and surrounding communities via the Family Health Center - Concord site in the Yeaple Building, adjacent to Concord Hospital, and its satellite Family Health Center - Hillsboro-Deering located approximately 20 miles west of Concord in the town of Hillsboro.

Family practice residents are assigned to a panel of family and prenatal patients with whom they work for the length of their residency. Residents visit their patients in various settings including the hospital, nursing homes, and at-home as necessary.
In addition to faculty and residents, the Concord Hospital Family Health Center - Concord and Family Health Center - Hillsboro-Deering staff includes business staff, physician assistants, nurse practitioners, nurses, medical assistants, behavioral health counselors, social workers, clerical workers and a nutritionist.
