In situations where patients are confronting serious and life-threatening illnesses, a member of the inpatient or outpatient care team, including physicians, nurses, social workers, spiritual care counselors, dieticians, and pharmacists, will meet with them and their loved ones, to ensure they are connected to programs and services that best meet their needs.

A multidisciplinary team of physicians, nurses, social workers, spiritual care counselors, hospital volunteers, and pharmacy and nutrition consultants provide patients the medical care they need and frequently reassess their needs to ensure the most comprehensive, skilled, and sensitive care is offered.
A skilled and responsive interdisciplinary team is committed to working with patients to relieve pain, suffering, and promote empowerment. Goals for treatment are mutually agreed upon with the patient and family to provide relief and to maintain communication with their surroundings.
Patients nearing the end-of-life may experience feelings of fear, isolation and anxiety. Our skilled care team monitors and addresses such feelings; to ensure patients care plans are adapted to meet their personal needs. Staff, trained in end-of-life care, is available to answer and address the many questions, concerns, and emotional feelings that may surface. Additionally, trained individuals offer patients a variety of other services including meditation, prayer and Reiki, as well as the companionship of a caring person available to offer a listening ear.
Although most hospice care is provided at home, Concord is fortunate to have the Hospice House, administered by the Concord Regional Visiting Nurse Association and located on the Concord Hospital campus, for those patients needing support and caring beyond what their loved ones can provide at home.
