End-of-Life

Call (603) 227-7000, ext. 3013 for more information.

A branch with budding white flowersWe are dedicated to preserving and enhancing, when possible, the quality of life for patients and families. In situations where patients are confronting serious and life-threatening illnesses, a member of the inpatient or outpatient care team, will meet with them and their loved ones to ensure they are connected to programs and services that best meet their needs and frequently reassess their needs to ensure the most comprehensive, skilled, and sensitive care is offered.

Staff, trained in end-of-life care, is available to answer and address the many questions, concerns, and emotional feelings that may surface. Additionally, trained individuals offer patients a variety of other services including meditation, prayer and Reiki, as well as the companionship of a caring person available to offer a listening ear.

Although most hospice care is provided at home, Concord is fortunate to have the Hospice House, administered by the Concord Regional Visiting Nurse Association and located on the Concord Hospital campus, for those patients needing support and caring beyond what their loved ones can provide at home.
