About Us

Concord Hospital's Diabetes Self-Management Program provides training and self-management education to people with diabetes and their families. The program is a comprehensive outpatient referral-based diabetes education program that empowers patients to take control and successfully manage their diabetes. By participating in our program, patients have an opportunity to fully understand the rationale for early aggressive intervention to prevent complications from diabetes mellitus.

Our certified diabetes educators work closely patients' physicians or primary care providers. Educators conduct a thorough patient assessment to recommend safe and appropriate medication and/or insulin requirements based on pattern management. Physicians receive a summary of the visits and patient specific goals.

Patients of Diabetes Self-Management have access to:

  • Pre-diabetes and prevention of diabetes
  • Basics of Type 1, Type 2 and gestational diabetes
  • Blood glucose monitoring
  • Nutrition counseling for blood glucose and lipid management
  • Guidelines for meal planning, dining out, grocery shopping, label reading and travel
  • Insulin and medication
  • Diabetes and sick days
  • Acute and chronic complications
  • Stress management
  • Behavior goal setting
  • Preconception care and gestational diabetes
  • Introduction to pump therapy
  • Glucose sensor
  • Exercise program based on your fitness level, interest and needs

Group Classes

The more patients know about diabetes, the better they can care for themselves. The following group classes are available:

    Prediabetes Class - With the ever-increasing rise in diabetes, we are pleased to offer this class to our community. The goal of this class is prevention of diabetes. Minor lifestyle changes can help prevent ever becoming diagnosed with Type 2 diabetes.

    Living with Diabetes: Diabetes Self-Management Group Classes - Newly diagnosed patients with Type 2 diabetes are encouraged to attend these classes. Sharing experiences within a group and learning how other people cope with diabetes is great. Start off with the first class and then schedule the remaining three classes. This eight-hour class series provides participants with information required to begin self-managing their diabetes. The class topics include nutrition, exercise, monitoring, medications, complications, community resources and stress management. Classes are offered at various times.     Advanced Carbohydrate Counting - This class is recommended for those who wish to intensify their insulin management and for those who are considering insulin pump therapy.

    Introduction to Insulin Pump Therapy - This class is offered once a month to those who wish to learn about insulin pump therapy.
