Our Concord Hospital Medical Group (CHMG) consists of more than 245 primary and specialty providers specially trained in 17 subspecialties and conveniently located throughout central New Hampshire.
Our practices feature modern facilities and state-of-the-art technology, offering patients both convenience and comfort. For convenience, many of our practices are co-located with other medical services including imaging, rehabilitation and laboratory services.
Electronic Health Record (EHR)
One sophisticated electronic health record (EHR) system is used throughout CHMG practices to enhance treatment and patient safety by giving providers a unified record for each patient, listing information such as medical problems, allergies and medications.
Patient Connect, the EHR's online patient portal, provides CHMG patients access to a free, secure service where they can efficiently manage their health information and communicate directly with their medical practice. More than 6,000 CHMG patients are checking personal health information, requesting appointments, renewing prescriptions, paying medical bills and communicating with their healthcare providers' offices through Patient Connect.
Patients are valued members of their treatment team. Our CHMG providers, staff and managers look to the CHMG Patient Advisory Forum, which includes CHMG patients, to learn about patient needs and to review patient comments to monitor what practices are doing well and how they might improve.
Additionally, our practices measure patient satisfaction through Press Ganey, an independent provider of satisfaction surveys and other satisfaction measurement tools. The surveys data is used to assess and improve performance of the practice.
Financial counseling and assistance is available to CHMG patients through Patient Financial Services and eligible patients can receive help applying for free medications through our Prescription Assistance Program.
Our Hospitalists Program provides comprehensive, individualized medical care for our hospitalized patients including pediatric patients. The Hospitalists role is to oversee your care while hospitalized which may include ordering laboratory and other tests, ordering medications, consulting with specialists, arranging for any additional services you may need after discharge and keeping your family medicine provider informed of your treatment plan.
