Dr. Cindi Croft of Family Care of Concord sees CHMG patients by appointment Saturday, 8 am to 7 pm or Sunday, 8 am to 5 pm. Patient Connect Patient Portal

Patient Connect - My CHMG Health Care, the EHR's free, secure Web-based portal allows you to send messages to your CHMG providers' offices, request appointments and medication refills, and pay medical bills.

Patient Satisfaction
