In an effort to provide efficient and timely service, as well as to protect patient privacy, we ask patients to respect the following prescription refill guidelines:

• Bring a list of your medications, including over the counter medication and vitamin supplements, to every visit;
• Give 24 hours notice for prescription refills;
• Give 48 hours notice for controlled substance refills (i.e., narcotics and ADHD drugs);
• Mail order precriptions are a patient's responsibility. Our providers will provide a written prescription for you to submit to your mail order pharmacy.
• Be prepared to show photo identification when picking up prescriptions or medication samples;
• If someone other than the patient is picking up a prescription, he/she must be 18 years or older and must be prepared to show photo identification.

Prescription refills require:

• Name and date of birth;
• Name of medication;
• Dosage directions;
• Supply designations (i.e., 30 or 90 day supply);
• Pharmacy name and location;
• If requesting written prescription, specify to pick up.
