We know involving you and your family in your care can improve communication and outcomes. Toward that goal, "rounding teams" have been implemented on some patient care units and include the patient's family, hospitalist physician, pharmacist, registered nurse (RN) care manager, nurse manager and the RN caring for the patient.
The team meets bedside Monday through Friday to review the reason for hospitalization, that day's plan of care and plan for discharge. The team hears and understands the patient's perception of their illness and identifies appropriate resources that may be needed after discharge. The meetings avoid miscommunication and multiple conversations regarding their needs and improve the efficiency and timeliness of medications.
The result has been higher Press Ganey patient satisfaction scores and positive comments from patients, including: "The rounds of all staff together were very good to be sure that were all on the same page."
