Gail Fayre MD
Program Director and Assistant Professor of Community and Family Medicine
Dr. Fayre joined the NHDFMR faculty in July 1996 after spending two years on the family practice residency teaching staff at Erhling Berquist Hospital in Nebraska. Her clinical experience includes two years at the 39th Medical Group Hospital in Turkey. Dr. Fayre's clinical interests include obstetrics, sports medicine and outpatient procedures.

J. Bryan Bannister MD
Dr. Bannister received his medical degree from the University of Texas Medical Branch, Galveston in 1998. He completed his family medicine residency at the University of Rochester. Dr. Bannister joined the NHDFMR faculty in 2007 and oversees the inpatient medicine curriculum.

Suzanne Boulter MD
Adjunct Assistant Professor of Pediatrics
Dr. Boulter is a pediatrician with 30 years experience in providing care for children in the Concord community. She joined NHDFMR in 1997 to pursue public health advocacy projects for children and teens. She practices at the Family Health Center in Concord and precepts residents in the care of children and teens. Some of the projects she has been involved in include developing health and wellness services for teens and establishing Concord Hospital's "Reach Out and Read" pediatric reading program.

Teri Brehio MD
Dr. Brehio joined the NHDFMR faculty in September 2004. She graduated from the University of Massachusetts Medical School and completed her residency at Concord Hospital, where she served as chief resident from 2003-2004. Dr. Brehio practices at our Hillsboro clinic and is the rotation coordinator for the orthopaedics/sports medicine and ICU curricula. Her clinical interests include obstetrics, teen care, preventive medicine, and dermatology.

David Conway MD
Assistant Professor of Community and Family Medicine
Dr. Conway began his career as a registered nurse, working as a labor and delivery nurse from 1980 to 1982, before attending Albany Medical College. He completed his OB/GYN residency at Hartford Hospital in 1990. He worked in a large group private practice in Portland, Maine from 1990 to 1992, then in a rural solo practice in St. Marys, Pennsylvania from 1993 to 2001. Dr. Conway provides OB/GYN consultations for the family medicine faculty and residents in general obstetrics, gynecology and infertility as a member of the faculty.

Mary Danca MD
Education Director
Assistant Professor of Community and Family Medicine

Dr. Danca received a master's degree in human genetics in 1976 and spent several years as a genetic counselor in Portland, Ore. She then completed her medical training and was in private practice in OB/GYN for 13 years before joining NHDFMR. She provides consultations in obstetrics and gynecology and serves as an education director. Additionally, Dr. Danca coordinates the surgical subspecialty rotation.

George (Skip) DeVito MD
Adjunct Assistant Professor of Pediatrics
Dr. DeVito, a primary care pediatrician, has worked in Concord for the past 30 years. He became affiliated with the NHDFMR in 1995 and joined the program as a full-time faculty member in 1999. Dr. DeVito is Chair of the Department of Pediatrics at Concord Hospital. His hospital responsibilities include pediatric ward attending and didactics, developing a community-based pediatric consultative service, and leading initiatives to improve pediatric care at Concord Hospital. Dr. DeVito oversees the pediatric outpatient curriculum. He leads the pediatric behavioral assessment clinic, supervises the educational well child clinic, and has developed a pediatric medical home clinic for complex pediatric patients. His teaching interests include neonatology, well child care, healthcare for special needs children and their families, and pediatric behavioral problems. Dr. DeVito is two-time recipient of NH Magazine Top Doc Award in pediatrics. He was named NH Pediatrician of the Year by the NH Pediatric Society in 2007.

Doug Dreffer MD
Medical Director
Assistant Professor of Community and Family Medicine

Dr. Dreffer joined the NHDFMR faculty in 2000. He currently serves as Medical Director of the Concord Hospital Family Health Centers located in Concord and Hillsboro. He earned his medical degree at the Ohio State University College of Medicine and completed his residency at Concord Hospital. During his residency he served as chief resident from 1999 to 2000. Dr. Dreffer currently splits his clinical and precepting time between the two clinic sites. His teaching interests include obstetrics, sports medicine, and geriatric home care.

Beverly (BJ) Entwisle MD
Assistant Professor of Community and Family Medicine
Dr. Entwisle trained in Primary Care Internal Medicine at Boston City Hospital. She entered group practice in 1988 in Concord where she pursued an interest in geriatrics. She obtained her Certificate of Added Qualification in geriatrics in 1994. She joined the NHDFMR part-time in 1995 to teach geriatrics, while continuing her primary care practice. In 1998, she became a full-time faculty member. Dr. Entwisle teaches all aspects of adult medicine. She has developed the program’s Frail Elder Program, which includes nursing home visits and home care services; these function as sites for geriatric teaching. Dr. Entwisle is also the current Medical Staff President at Concord Hospital.

Dan Eubank, MD
Director of Graduate Clinical Education Dartmouth Medical School Department of Community and Family Medicine
Dr. Eubank worked in a local group practice for 15 years before founding NHDFMR in 1992. He served as the medical director of the community hospice program for more than 10 years. Between 1984 and 1997, he served as a founding board member of a regional health maintenance organization. Additionally, he has developed local physician support groups and authored an article on this subject. Dr. Eubank’s teaching interests include psychosocial aspects of primary care, organizational dynamics and healthcare systems.

Dominic Geffken MD, MPH
Director of Preventive Medicine
Dr. Geffken joined the NHDFMR faculty in July 2004 as the director of leadership preventive medicine. He graduated from the University of Vermont College of Medicine. He completed both his family practice residency and preventive medicine residency at the University of Massachusetts-Worcester. He also received his Master of Public Health degree from the University of Massachusetts-Worcester in June 2004. His interests include preventive medicine, behavioral health, and geriatrics.

Laura Greer, MD
Dr. Greer received her medical degree from Albany Medical College in 2006. She completed her pediatric residency at Children’s Hospital at Dartmouth Hitchcock Medical Center. She joined the NHDFMR faculty as a pediatric hospitalist in 2009. Her clinical interests include infectious diseases and pediatric vaccinations.

Bill Gunn PhD
Assistant Professor of Community and Family Medicine
Dr. Gunn, a licensed family psychologist, joined NHDFMR in 1997. He received his doctorate in family therapy from Virginia Polytechnical Institute and State University and has 20 years experience teaching behavioral medicine at family medicine residencies in Virginia, Colorado and North Carolina. Dr. Gunn co-authored a book on models of collaboration between psychotherapists and physicians in 1996 and recently published another book on the same topic. He has collaborated with the physician faculty to create an innovative, longitudinal curriculum for residents. Dr. Gunn also works closely with organizational development at Concord Hospital to help train physicians as leaders in their communities.

Joni Haley MS, LMFT
Collaborative Care Service Manager
Ms. Haley, a licensed marriage and family therapist, joined NHDFMR in May 2000. She directs outpatient behavioral health services and the behavioral health internship program at the Concord Hospital Family Health Center - Concord. Ms. Haley supervises the masters-level and doctoral-level interns who are members of the NHDFMR interdisciplinary psychotherapy team. She also directs the collaborative pairings between behavioral health interns and residents in the clinic.

P. Travis Harker, MD, MPH
Dr. Harker joined our faculty in 2006. He is a graduate of our combined Family Medicine/Leadership Preventive Medicine Residency Program. As part of his training, Dr. Harker earned a Master of Public Health degree from Dartmouth College and focused his preventive medicine practicum on improving care for overweight and obese patients. He currently is the Chair of Quality Improvement for the Concord Hospital Family Health Centers and leads the quality improvement curriculum for the residents. He has worked to improve systems of care related to weight management, colorectal cancer screening, and oral health. His clinical interests include weight management, quality improvement, motivational interviewing and full-spectrum family medicine.

Alison Holmes MD, MPH
Assistant Professor of Community and Family Medicine
Dr. Holmes received her medical degree from Stanford Medical School in 2000. After completing a pediatric residency at the University of North Carolina, Chapel Hill in 2004, she continued training as a general pediatrics fellow at the University of Rochester in Rochester, New York. There she received her Master of Public Health degree in 2006. She joined the NHDFMR faculty as a pediatric hospitalist in 2006. Her interests include breastfeeding from both public health and medical perspectives and pediatric hospital medicine - developing excellent systems of care for pediatric inpatients.

Tina Kenyon ACSW
Instructor of Community and Family Medicine
Ms. Kenyon joined the NHDFMR faculty in 1995. As part of a team, she coaches residents through the process of completing longitudinal scholarly projects. Ms. Kenyon contributes to resident advising/support, didactic education, curriculum coordination, and medical student integration. She facilitates groups for faculty and residents to explore clinical uncertainty, and a resident Balint group. Ms. Kenyon enjoys promoting scholarly activity, including conference presentations and publications. Prior to her current role, she taught behavioral science at the Brown Family Medicine residency for several years. She possesses an extensive background in clinical family therapy, facilitation, and coaching. Her academic interests include curriculum design, transformational learning, organizational evolution and community-based research.

Cynthia King MD
Dr. King joined our faculty in 2006. Dr. King received her medical degree from University of Massachusetts in 2002 and entered the University of Rochester Family Medicine Residency Program. She had the opportunity to serve as chief resident from 2005-2006. Her interests include patient and resident education, international and migrant health and women's health.

Ursula Kneissl MD
Adjunct Assistant Professor of Pediatrics
Adjunct Assistant Professor of Pediatrics Dr. Kneissl, a pediatrician, began working with our residents while practicing in 1996. She left practice to join the NHDFMR faculty in 1999. As a pediatric hospitalist, she is primarily involved in teaching all aspects of inpatient pediatric care, including acute care and management of ill neonates. She also provides outpatient consultation on pediatric cases for area physicians. Her involvement at Concord Hospital includes quality improvement for special medical needs children, pediatric sedation and pediatric trauma care.

Mike Madan MD
Education Director
Dr. Madan joined our faculty in 2007 and serves as one of the education directors. He received his medical degree from UMDNJ in 1993. Upon completion of his family medicine residency at Maine Medical Center, Dr. Madan served as chief resident. He served as an Assistant Program Director at Maine Medical Center for several years and was the founding director at two family health centers in New England. Dr. Madan coordinates the dermatology and procedure curriculum. His clinical interests include sports medicine and osteopathic manipulative therapies.

Marianne McCall MD
Dr. McCall joined the faculty in 2009. She is a graduate of the University of Florida College of Medicine and completed her residency training at the Tallahassee Memorial Family Practice Residency Program. Following residency, Dr. McCall gained clinical experience in primary care, hospitalist medicine and emergency medicine. Her clinical interests are broad and she enjoys the full scope of practice that family medicine affords. Some of her special interests are in psychiatry, pediatrics and the treatment of myofascial pain disorders.

A. John Orzano MD, MPH
Dr. Orzano joined our faculty in 2008. He received his medical degree from Georgetown in 1973. Upon completion of his family medicine residency at Hunterdon Medical Center, Dr. Orzano served as adjunct assistant director at Somerset Family Medicine Residency Program while maintaining a private medical practice. His residency responsibilities included directing faculty development and resident scholarly activities. In 1995, Dr. Orzano assumed a full time faculty position at Robert Wood Johnson Medical School to concentrate more on practice-based research activities. After completing a Post-Doctoral Fellowship in Primary Care Research and MPH degree, Dr. Orzano assumed various duties including principal investigator on NIH and foundation funded practice improvement projects. Dr. Orzano’s interest is in using his broad background to mentor residents and faculty to create and maintain evidenced-based, relationship-centered healing environments.

Gregory Thesing MD
Assistant Professor of Community and Family Medicine
Dr. Thesing joined the NHDFMR faculty in 1997 after spending two years as a staff physician at the United States Air Force Academy. Additionally, he was a clinical instructor in the Department of Family Medicine at the University of Colorado Health Services Center. Prior to his work in Colorado, he spent three years as staff family physician at Eielson Air Force Base in Alaska. Currently, he practices at the Family Health Center in Hillsboro. His teaching interests include outpatient procedures, precepting and obstetrics.

Angela Yerdon McLeod DO
Assistant Professor of Community and Family Medicine
Dr. Yerdon received her osteopathic medical degree from the New York College of Osteopathic Medicine in 2001. Following her family medicine residency at Concord Hospital, she completed a fellowship in Women's Health at East Carolina University in Greenville, NC. She then returned to NHDFMR to join the faculty in 2005. Currently, Dr. Yerdon McLeod is the maternal child health curriculum coordinator and has successfully introduced Centering Pregnancy group visits into our clinic. Her clinical interests include family centered maternity and neonatal care and women's health including gynecological procedures and prevention of chronic disease.
