Jonathan Ballard, MD
Jonathan Ballard is the recipient of the Society of Teachers in Family Medicine's H. Winter Griffith Resident Scholarship for Excellence in Practice Improvement and Patient-centered Care. As one of our combined Family Medicine/Leadership Preventive Medicine residents, Jonathan's practicum project has focused on improving the care of patients with cardiovascular disease through a team-based and patient-centered approach within the Green Pod of the Family Health Center. The Green Pod team working with their patients have improved the rates of blood pressure control, cholesterol management, smoking cessation, and adherence to recommended aspirin therapy guidelines over the course of Jonathan's quality improvement practicum. Jonathan will present his work at STFM’s Practice Improvement Conference in San Diego.
The combined Family Medicine and Leadership Preventive Medicine program has been an opportunity for Jonathan to learn how to measure quality outcomes in medicine and to then act to improve the outcomes through designing and implementing a quality improvement initiative. Jonathan credits the success of the practicum to the great teamwork and collegiality present at the NH Dartmouth Family Medicine Residency and at the Concord Hospital Family Health Center. Jonathan states "I truly appreciate the caring environment and dedication to the work of family medicine by all of the faculty, staff, and residents at the residency. It's been the perfect place for me to train and learn throughout residency."
